FREE NHS HEALTH  CHECK
Ages 40 - 74 years?


You may be entitled to a fee NHS health check.  This health check can help to detect diabetes, high blood pressure and high  cholesterol . The check will also assess your risk of developing cardiovascular disease in the future and with the right measures can prevent these diseases.

 

To qualify for your health check you must be aged between 40 - 74 years and NOT ALREADY receiving treatment for: Heart disease, Kidney disease, stroke or diabetes

 

To arrange your free health check please contact the surgery on 01257 791216 and ask for an appointment for your diabetes and cholesterol blood test followed by an appointment with our health care assistant

If you do not require an urgent appointment you are requested to contact the surgery after 9.30am

We work in partnership with other practices within the Chorley area to offer routine appointments with a doctor during the evening and at weekend / bank holidays (subject to availability). You can book these appointments by contacting our reception / care navigation team on 01257 514240. Please be advised these appointments will require you to travel to an alternative medical centre in central Chorley.

Routine appointments are for those whose condition is not urgent and can wait until the next routine appointment is available. Routine appointments are usually available to book in advance and you may ask to see the clinician of your choice (subject to availability).

Routine appointments can be booked directly with the surgery or online. Do not book a routine appointment if you feel your case is urgent.

 

Urgent appointments are available for urgent conditions that require assessment the same day or can not wait until the next available routine appointment.  You may not be able to see the clinician of your choice.  Urgent appointments must be booked directly with the receptionist.  The doctors require that the receptionist ask you some questions to prioritise your case, please co-operate with this as it is necessary.

Urgent appointments are only released on the day and can not be pre-booked. As urgent appointments are at the end of each clinic session we can not negotiate appointment times, you will need to attend at the time given. You are requested to ring before 10.00am for an urgent appointment.  You must tell the receptionist if you require an urgent appointment otherwise you may be offered a routine appointment.

 

How you can help us:

  • Be on time for your appointment
  • Tell us if you need to cancel (you can cancel any appointment online, contact reception to register for this)
  • Call before 10.30am for a home visit 
  • Call as early as possible for an urgent appointment 

If your appointment is for an annual review with the Practice Nurse please arrange this appointment in good time as these are non-urgent appointments and can be booked well in advance to avoid delay.
