FREE NHS HEALTH  CHECK
Ages 40 - 74 years?


You may be entitled to a fee NHS health check.  This health check can help to detect diabetes, high blood pressure and high  cholesterol . The check will also assess your risk of developing cardiovascular disease in the future and with the right measures can prevent these diseases.

 

To qualify for your health check you must be aged between 40 - 74 years and NOT ALREADY receiving treatment for: Heart disease, Kidney disease, stroke or diabetes

 

To arrange your free health check please contact the surgery on 01257 791216 and ask for an appointment for your diabetes and cholesterol blood test followed by an appointment with our health care assistant

 

 

New Patient Registration

registrationWe accept new patients who live within our practice area which is Coppull and parts of Charnock Richard.  As it often takes sometime for records to be forwarded from your former practice all newly registered patients are required to complete a health questionnaire and if you are 14 years or over your will need an appointment with our health care assistant for a new patient health screen. 

If you are taking repeat medication you will also need an appointment to see the doctor so your medication can be prescribed. Please ensure you see the Doctor before your medication is due to run out.

Medical treatment is available from the date of registration.  Please collect a registration form from reception.
